It seems like a wrong make4ht installation. It cannot load any extensions. You may try instead this: tex4ebook -c mycfg.cfg draft "svg" It should still produce SVG images for PDF files, but the compilation will be slower. The generated Epub file is quite large, here is a link:

What problems do happen? MathML shouldn't be used unless you explicitly use the "mathml" option. There are some notorious issues with sub and super scripts with the MathML option, TeX4ht needs to enclose the base and exponent in {} in order to work correctly, for example. I am sure there are
