I think I may have narrowed down the issue. I changed the cupsd.conf
LogLevel to 'debug'.
When adding a printer via SMB with 'requires authentication' ticked and
filled out I noticed that the logfile says:

I think the current one I have right now is the default Ubuntu one so

I think I am seeing the same issue on my little server.
Screen goes blank. No mouse response.
Can''t switch VC's, CTL-ALT-BS, or CTL-ALT-DEL.
Not accessible from network either.
Next time it happens, I'll try doing a ALT-SysRq-E and dig around.

A little update.
I have been testing Fedora rawhide and noticed that their nodoka gtk
engine (originally based on murrine, I believe) uses the same fix as I
posted in this bug for Ubuntulooks and Murrine (originally found by
Andrea Cimitan). The issue with GtkEntry boxes is more complicated and

packages in 'universe', but may not depend on packages in universe, not
even for build deps. Looks like we're going to have to try and put jack

You can have it work by specifying the Orientation in every printers in Cups
System - Printing
For each printer select Job Options tab and specify Orientation to whatever
else instead of Automatic Rotation.

I have the same problem, on 2.6.24-16-generic, Hardy beta.
ls in /media, for example, returns the following (among other things):
