Sorry, but I can't collect the logs with nvidia drivers since the nouveau are
automatically loaded when booting to the live CD and I can't unload nouveau
drivers to let the nvidia drivers work.
X still fails to run with nvidia (173) drivers on a fully updated 13.10 system.

non-linux machine connected to the same USB hub. I wrote this problem
off to be a hardware issue of my USB hub and bought a new one and
haven't seen this problem since...

Ok, here's a good news. I believe I have a code change to overcome chip
bug.
A bit of debug description:
I collected couple of more win7 IOMMU traces. It turns out even win7
sometimes does not detect mask in first place. It did about 20
read/writes before reading mask again.
