I can confirm this bug on my:
Compaq C700
CPU: Intel Pentium Dual CPU T2310 @ 1.467GHz -
GPU: Intel Mobile GM965/GL960
OS: Ubuntu 14.04 trusty
Kernel: i686 Linux 3.16.0-38-generic
I DO NOT have the problem on my:
Lenovo Thinkpad Edge E125
CPU: AMD E-450 APU with Radeo HD Graphics @ 1.65GHz

Problem solved.
The bug is not in the ubuntu-docs - it's with this user :)
When installing phpmyadmin in the Configuring phpmyadmin dialog box i
read the red cursoricon before apache2 as it was selected. I didn't hit
space to actually select apache2. When I do that I get the same output
