I'm here to report a similar issue. But what distinguishes my case from
all of you is that I formatted 16.04 straight from live media. I never
upgraded from 14.04.
I'm getting this:
$ sudo apt-get upgrade

@ironstorm, like you, my /etc/modprobe.d/pptpd.conf doesn't exist and
/lib/modules-load.
By the way, my initial error was: libkmod: ERROR ../libkmod/libkmod-
config.c:635 kmod_config_parse: /lib/modprobe.d/pptpd.conf line 1:
ignoring bad line starting with 'nf_nat_pptp'

Thanks for the clarification, psusi. After aborting the incomplete
installation procedure, upon reboot 16.04 booted just fine and the only
other O/S on my SSD was Windows 7, all of which is perfectly accessible
now, even with an EFI mode installation (that could have been BIOS mode
instead).

TestDisk doesn't recognize any problems now and gparted and ubiquity
still recognize nothing. I have been without Ubuntu now for days. This
is really frustrating for me. =/

Ah ha! playback is only messed up when compiz fusion is running. If I
disable compiz fusion, playback is fixed. Therefore, this is a bug
with compiz fusion, not gstreamer.

I don't know what causes it, but after a few hours of using Ubuntu
Gutsy, wmv, avi and mpeg playback fails. I hear sound, but I can't see
the video. All i see in my Totem media player or MPLayer is pink and
yellow gibberish. My current work around is to restart x w/ CTRL + AL
