I can confirm that this bug is still present in Ubuntu 12.10.
Installation goes fine, up until all files are copied and the installer
goes to Installing system where the slideshow rolls.
The workaround to remove the ubiquity-slideshow-ubuntu package

The workaround in post #7 no longer seems to work for my Ubuntu 12.04
x86_64 system, as of thunderbird package version 11.0.1+build1-0ubuntu2.
After upgrade, thunderbird immediately jumps to a Mozilla bug reporting
screen on start. I've ran this command as suggested:
cp

something that I could fix my Precise installation myself? Or is this
something the Ubuntu Pulseaudio team could release a fix for? Or does it
need to be reported upstream?

When a Plantronics DA45 adapter is plugged into the system, Xorg
segfaults while initialising evdev. See attached Xorg log.
This happens in Ubuntu 12.04. I did not experience the same problems in
11.04.

Importance:
I have worked around this problem using the following in xorg.conf:
Section InputClass
Identifier Plantronics Plantronics DA45
MatchProduct Plantronics Plantronics DA45
Option Ignore on
EndSection
This disables the buttons on the headsets, but not the audio device

or while printing to a file. I will file a new bug report per your

Changing LanguageLevel from 3 to 2 in the PPD file remedied this problem
for me.
Updating with more information: I have just experienced this exact same
issue with a Plantronics DA45 adapter (I'm assuming it's similar to the
DA40), this time with Natty Narwhal (11.04).
