Problem exists for me as well on Powertop versions 1.12 and 1.13. It
does not exist for Powertop version 1.11.

Argh. I spoke too soon. Powertop 1.11 monitors for 5 seconds, displays
the results, and then there is a segmentation fault. So, it gets
further, but it also crashes. At least it doesn't take the whole system
down with it. ;)

I downgraded to Ubuntu 8.10, and my large block size transfers (between
2MB and 3MB) are working correctly now.
Based on my experience, I believe the qla drivers may be broken in 9.4,
for block sizes larger than 1kB.
It looks like none of the developers are following this bug report,
maybe
I have two Ubuntu systems, one is 8.10 and the other is 9.04 (upgraded
from 8.10).
The 8.10 system has no trouble with the following QLogic FC cards:
QLE2460 (4Gb)
QLE2562 (8Gb)
However, neither of these cards work properly in the 9.04 system. My
dmesg output looks similar to that above, in

Please note that I don't need to boot from the FC device. I just need
data transfers to work properly, and they don't.
I have tried
sudo rmmod qla2xxx
sudo modprobe qla2xxx
But, this doesn't fix the data transfer issue.

Setting Emulate3Buttons to false no longer fixes this problem in 8.10.
I'm guessing this has something to do with evdev.

Yes, evdev is being used:
$ grep evdev /var/log/Xorg.0.log

I managed to work around this problem creating an .fdi file in
/etc/hal/fdi/policy to set Emulate3Buttons to false
You have to be in text mode for the kernel panic to display. e.g. press
Ctrl-Alt-F1. I tested this on Feisty, but I now have a broadband
internet connection so I don't need to use a dial-up modem. I do still
have the modem and dialup account, so I'll try reproducing it on Gutsy.

I get a kernel panic every time I make a dial up connection to certain
ISPs. The panic always happens within a few minutes of connecting. I've
reproduced this consistently on various Linux distributions and
different hardware. I currently use Ubuntu, so that's why I'm
