In my case, I was getting this error because I didn't have a PolicyKit
authentication agent running. Launching `/usr/lib/policykit-1-gnome
/polkit-gnome-authentication-agent-1` before running usb-creator-gtk
fixed this issue for me.

After 4 Kernel updates it is full, and very hard for non technical users

I'm migrating from a laptop running 14.10 to another running 15.04.
0. On 14.10 machine
1. click network manager icon
2. click 'edit connections'
3. select VPN network and click 'Edit...'
4. select 'IPv4 Settings'
5. click 'Routes...'
6. add a route
7. click 'ok'
8. click
I had this problem with 3.19.0-18-generic, and reading this bug prompted
me to reboot and select instead 3.19.0-16-generic, and now I see hdmi
audio as an option again, and it works fine.

I see this with VLC too, so it's probably with any fullscreen app.
However, at least on 15.04, ticking the 'Always on top' option via the
right-click titlebar menu of the video window is an effective
workaround.
This seems to suggest it ought to be easy to fix? It's clear Unity can
draw things
