I'm seeing this bug, typically related with inserting a smartcard
reader, AND THEN inserting the smartcard:
[12030.538992] usb 1-1: new full-speed USB device number 6 using xhci_hcd
[12030.668391] usb 1-1: No LPM exit latency info found, disabling LPM.
[12030.676665] usb 1-1: New USB device
Oops, I forgot to say: if happens with ubuntu 15.04, kernels 3.19
(standard package), and 4.0.* and 4.1-rc{2,3} from mainline.

Regarding the problems described starting at comment 40 on 2013-03-27, with
exception of comment 41,
I made fixes for them and submitted them upstream. Please consider backporting
them to ubtuntu kernels.
staging: rts5139: Use correct USB transfer interval to fix syslog spamming:
The rts5139 driver is responsible for both, the messages and also the
non-working SD card slot.
After sudo rmmod rts5139 the warnings that were printed every 5
seconds will stop.

Log full of xhci_hcd WARN
To manage
I'm running 3.7.6 on a Samsung Chronos 7 (780Z) laptop and get flooded
with these:
[28568.301952] xhci_queue_intr_tx: 74 callbacks suppressed
[28573.414409] xhci_queue_intr_tx: 74 callbacks suppressed
[28578.526801] xhci_queue_intr_tx: 74 callbacks suppressed
[28583.639180] xhci_queue_intr_tx: 74
I also found a few of these, which is identical to the error the
original poster was receiving:
xhci_hcd :00:14.0: WARN Event TRB for slot 1 ep 6 with no TDs
queued?

Updated kernel to 3.8.4 (latest stable; dated 2013-03-20) and receive
the same errors as on previous versions (every 5 seconds):
[ 11.482307] xhci_queue_intr_tx: 71 callbacks suppressed
[ 16.594634] xhci_queue_intr_tx: 76 callbacks suppressed
[ 21.707011] xhci_queue_intr_tx: 72 callbacks
Using latest upstream mainline kernel (3.7.0 rc1), the problem seems to be
fixed.
I now only get a serie of warning when doing a lsusb -v
Otherwise, it seems to not happen (but maybe I'm not using the device that
trigers the warning..)

This issue appears to be an upstream bug, since you tested the latest
upstream kernel. Would it be possible for you to open an upstream bug

Also, do you happen to know if this issue happened in prior releases,
