Same situation here. I believe in my case, this is triggered by turning effects On (System Preferences Visual Effects), and subsequently turning Visual Effects back off. While 'On', ALT key combos work fine. Then when turned off, ALT is ignored. Pressing ALT-TAB twice when focus is on a terminal
