As per Notification Desktop Specifications and NotifyOSD Specs², if a
notification provides a "replace_id", and a notification with that id is
shown or in queue, we should replace that with the last one.
This is something that notify-osd doesn't do. Breaking the protocol
apparmor 2.10.95-0ubuntu2 might fix this issue (Bug #1569316)

The errors there look like the issue would be about NetworkManager not
running. Are you sure it is started at the time? You wouldn't then have
to restart nm-applet -- nm-applet does not show if it can't talk to NM.
Otherwise, I suppose there may be issues with policykit or some other

As far as I can see Ubuntu only supports 2 release upgrade paths[1]:
- 1. ${release} upgrade to ${next-release} (e.g. for 14.04 -> 15.10, you'll
have to 14.04 -> 14.10 -> 15.04 -> 15.10, 3 upgrades in a row)
- 2. ${lts-release} upgrade to ${next-lts-release}
I ended here as this bug occurred for me also. I'm running Ubuntu Gnome
16.04, just pointing it out as it says it's already fixed.
