This concerns shim 13-0ubuntu2 in Ubuntu 18.04/bionic.

(Note: I am not entirely clear on whether this issue belongs to shim, or
to grub2; please redirect as appropriate.)

I am installing Ubuntu with EFI support with the following two
prerequisites:

  1. No changes are made to NVRAM (the system boots via e.g. "ATA HDD0"
instead of a dedicated boot option);

  2. The EFI removable media path (BOOT/BOOTX64.EFI) is used. (This is
kind of required by #1)

I have confirmed that this arrangement can be booted in Secure Boot mode
if the following two changes are made:

  1. BOOT/fbx64.efi is removed, to eliminate boot-loop behavior (same
issue as in https://launchpad.net/bugs/1750351, only unlocking the boot
order is not an option), and

  2. grubx64.efi and grub.cfg are copied from ubuntu/ into BOOT/ (as
BOOTX64!shim otherwise complains about not being able to find grubx64).

I would like for it to be possible to install Ubuntu in Secure Boot mode
in this manner, as the current approach effectively negates the intent
of the update_nvram=false debconf selection.

But once the base-installer comes up, the proxy is trying to be used. Hence the

The error in your log shows that your system is not booted in EFI mode,
and you do not have an EFI system partition.  So why are you trying to
install the shim-signed package?  Is this happening as part of a fresh

in this file, but it should be removed.

I did not choose a page size.  I used the defaults, which should have
been US Letter, as my locale is en_US.UTF-8 and my printer was purchased
in the US, and the files to be printed are formatted as US Letter.

In the printer properties, this is displayed as Printer Options -> Media
Size: Letter 8.5x11in

You keep saying these random behaviors. But I am unable to reproduce any
of them on 18.04 & 18.10. Can you explain, how to reproduce in  a
concise way?

And about sound indicator staying red after music stops, as I explained
it is intended behavior. Once a explicit user action is requested, the
program shouldn't do anything until that specific user action is
completed. This is in-line with ux design & concept (I discussed this
with previous dev). So I can't change anything in this area.

For instance, if you look at the apb.html to find d-i commands to
disable the network, then you'll see there is a netcfg component to
disable the network. However, the UbiquityAutomation wiki clearly says
that the netcfg component is not used by ubiquity. This led to the hack
in our 16.04 preseed file where we point the apt update server to a
bogus address to prevent auto-upgrades from occurring at install time.

Now that we are starting to look at creating an 18.04 desktop image,

Most of this will be copy/paste from that bug, as they still seem to be
issues in 18.04.

#2 and #3 we've figured out. #8 I haven't tried on 18.04, so I'm not
sure if it's still an issue.

There is no good or up-to-date documentation regarding how to modify and debug 
the Ubuntu 16.04 Desktop preseed file. The wikis, such as 
wiki.ubuntu.com/Ubiquity and all the wikis that link to/from it, do not give 
much help.

For example, some questions I have are:
1. What are the mappings of d-i keys used in the Ubuntu Server to ubiquity keys 
in the Ubuntu Desktop?

2. If I have both a ubiquity/failure_command and
ubiquity/success_command, and neither run, how do I debug this?

3. For OEM installs (oem-config/enable=true), how do I directly call
oem-config-prepare so that I do not have to manually double click on the
"Prepare for shipping to end user" icon? If your answer is "use the
ubiquity/success_command", see question #2.

4. Is it possible to use d-i directly instead of ubiquity for Ubuntu
Desktop installs?

5. Please provide documentation for installing additional packages from
the /cdrom/pool during install time. I don't want to have to unsquash
the casper/filesystem.squashfs, chroot into it and add packages that
way. I would like to be able to run "apt-get install <.deb file in
pool>" in the ubiquity/success_command.

6. During oem-config, I see a "Error opening file..." error pop up after
oem-config has finished asking questions, and starts installing.
However, I can't tell what it failed to open from the oem-config.log
(attached).

7. I am unable to disable the automatic upgrades during install time.

For instance, the kernel included with the 16.04.1 Desktop ISO is
4.4.0-31, while the latest in the Canonical repos (as of writing this
comment) is 4.4.0-64. I've tried adding the following into my preseed
file to try to prevent automatically upgrading the kernel during
ubiquity install time:

 d-i netcfg/enable boolean false
 d-i pkgsel/update-policy select none
 d-i pkgsel/upgrade select none
 d-i base-installer/kernel/image string none
 d-i base-installer/install-recommends boolean false
 d-i mirror/http/proxy string
 d-i apt-setup/use_mirror boolean false
 d-i mirror/protocol string
 d-i ubiquity/download_updates boolean false
 ubiquity ubiquity/download_updates boolean false

If there is a network connection, then ubiquity will automatically go
and update the kernel to the latest version. Worse yet, if there is a
network connection, but to one that can't reach the Canonical repos, it
will take an awful amount of time trying to download packages and timing
out. This is the case in the factory, where systems will be connected
only to a private network for PXE.

I want the initial state of a freshly installed system, whether it is
installed with or without a network connection, to be deterministic.

8) A PXE install where the PXE server and client are on a private
network with no external internet access will fail with a "The 'grub-
efi-amd64-signed' package failed to install into /target/. Without GRUB
boot loader, the installed system will not boot." error.

The PXE server initiates the install with the following grub boot menu
entry:

Where "(nd)/DevBoxSW/" is where the PXE server's TFTP server keeps a
copy of the vmlinuz and initrd of the ISO I'm trying to install,
"10.20.187.10" is the PXE server's NFS server, and
"nvidia/content/DevBoxSW" is the directory it exports that contains the
contents of the ISO file I want to install.

However, using the exact same ISO will work fine if I create a bootable
USB stick with it, and directly install on the client system where its
network cable is both plugged in and unplugged.

This concerns grub-efi-amd64 2.02-2ubuntu8.1 in Ubuntu 18.04/bionic.

When the grub-efi-amd64 package is initially installed on a system that
has not previously booted via the Ubuntu EFI bootloader (so /boot/efi/
is either empty, or at least does not have a /boot/efi/EFI/ubuntu/
directory), the package postinst script fails to install anything into
/boot/efi/.

The postinst script, in fact, contains this conditional around the grub-
install invocation:

if [ "$bootloader_id" ] && [ -d "/boot/efi/EFI/$bootloader_id" ];
then

It's not clear why this is here. There might be a reason why you would
not want to install/update the bootloader if the directory for it isn't
already there. In my case, however, I am running the postinst script
(via dpkg-reconfigure) precisely to populate an EFI partition from
scratch.

I can work around this issue with a simple

# mkdir -p /boot/efi/EFI/ubuntu

before running dpkg-reconfigure, but this is far from obvious.

An additional problem with the conditional is that if it fails, no
message is given to the user that the postinst is omitting the GRUB
install, let alone the reason why. If the user misses the absence of the
grub-install message, they'll only find out something went wrong when
they attempt to boot using the newly-installed bootloader and it doesn't
work.

OK, next please run these commands and send us the resulting files:

  dmesg > dmesg.txt
  journalctl > journal.txt

And in case something is crashing please also do:

1. Apply the workaround from bug 994921.

2. Look in /var/crash for crash files and if found run:
 ubuntu-bug YOURFILE.crash
   and tell us the ID of the newly-created bug.

3. If step 2 failed then look at https://errors.ubuntu.com/user/ID where
ID is the contents of the /var/lib/whoopsie/whoopsie-id file on the
machine. Do you find any links to recent problems on the machine? If so
then please send them to us.

There, Colin Watson made an interesting comment:

So it would seem that the old way of bringing up the GRUB menu by
holding down the Shift key just doesn't work in this brave new UEFI
world anymore.

It would be interesting to hear if anyone is aware of a system that
boots in UEFI mode and *does* respond to the held Shift key. Currently,
I don't believe it's clear as to whether this is an architectural
limitation of UEFI (i.e. it can't be fixed), or if newer UEFI firmware
versions could potentially make held modifier keys usable.

It appears this was fixed prior to the release of 18.04. Zero reports in
18.04 onward.

Unfortunately we can't backport a fix to 16.04 without knowing what
fixed it.
