When trying to upgrade glpi from hardy to lucid, it tries to apply the
change in /usr/share/dbconfig-common/data/glpi/upgrade/mysql/0.72.1.
This includes the line, ALTER TABLE `glpi_tracking` ADD INDEX `date_mod`

Specific steps:
1. Launch empathy. It will bring up a wizard offering to create an account.
2. Press cancel. The contact list entry will appear in the window list and the
window picker.
3. Close the contact list window. Now the entry is gone as it should be.

I’ve upgraded empathy to the Lucid version, and the only change is that
in step 4 it doesn’t bring up the wizard. The contact list still
doesn’t appear in either window list unless opened through indicator-
applet.

I’m seeing the same problem.
One interesting thing to look at is /var/log/auth.log:
