Same here, Ubuntu 18.04.1, Rhythmbox 3.4.2-4ubuntu1.

Some stations regularly skip, others stop playing after half a minute.
The problem is less visible with crossfade between tracks switched off.

Thanks for pointing out uninstalling the shell extensions. I had the
same problem and switching off all custom shell extensions helped
instantly.

Basically, this also urges an audit of gnome-shell and associated
libraries, as this type of crash _could_ be a security issue triggered
by extensions (potentially untrusted thris party data/code).

Just to chime in, running 16.04.2 LTS on a Lenovo E540.
I had this behaviour in dual head operation with an external full hd display.
I recently changed to a pivot QHD display, that operates in portrait mode.
I am losing windows frequently after returning from a locked screen.
This is a rather annoying and repeatable behaviour:
- I open a few applications, say, thunderbird, firefox and a terminal
- I need to leave, activate screen lock
- I return, unlock
- thunderbird window is lost, the dot is still displayed in unity's dock
- the window dows not return when clicking the application icon
- only solution is to right-click the icon, close the app and re-open it

Would it be possible to reintegrate the old advanced settings dialogue?
I mean, this is really a show stopper if you're using Model M keyboards from a 
decent series that do not have a Windows key. On all my systems I have mapped 
Caps Lock to Super L to access the menus.

Most of the time I use apt-get for package management.
Sometimes I use synaptic, especially when looking for packages, optional 
installs, desccriptions, screenshots, etc.
I use the find dialog (Ctrl-F) very often.

The find dialog saves the last used value of Look in: between sessions.
This is an annoyance, causing irritation very often.
It would be much more usable if the Look in: field would be initialized to 
Description and Name once, at program start.

for opening STL files from nautilus.
Apperently, it doesn't.
Furthermore, the option for choosing arbitrary programs for opening files in 
nautilus has vanished after the upgrade from ubuntu 12.04 to 12.10. Bummer.

What I want to accomplish is, that Meshalb opens when I double-click on
an STL file in nautilus.

Fixed by throwing in a handful of additional mimetypes and by supplying
the file name to the binary, to actually open a doubleclicked file.

upgrading cdrdao to 1.2.3 eliminates the error message.
what happens now is that the mounted CD-ROM is not unmounted before copying to 
image.

trying to create an image file from a CD in the drive.

Expected behaviour would be for the copy operation to begin.

What happens is a dialog box which tells All required applications and
libraries are not installed and Please install the following and try
again:.

No helpful information is provided by the dialog, whatsoever. No missing
components are displayed.

Something along these lines should work. Patch goes against 0.126.6.1

Beware of the attached patch, it's just a quick hack that should make the 
windows resizeable.
I couldn't test it, because my system update is still running ;-)

Fixed

Resolution:
Auto-update settings are kept in /etc/apt.conf.d/10periodic.
There was also /etc/apt.conf.d/20auto-upgrades, perhaps a remnant of an older 
version of the updater's instrumentation.
Removing the 20auto-upgrades file restores the desired behaviour.

Perhaps it would make sense to remove/rename the 20auto-upgrades file in
the post-install hook?

Above behaviour still happens, even with 2.25.91 (on intrepid).
It seems to me that the data acquisition for file sizes wedges at some point 
(mostly JPEG files) and does not continue from there.

I compiled brasero-2.25.91 from jaunty on intrepid/amd64 (got no jaunty 
installation here).
This version does not expose the behaviuour and seems to operate correctly.

Binary package hint: brasero

When opening a saved project (all files there and intact), brasero shows
the loading progress bar ('Please, wait while the project is loading.')
up to approx 98% and hangs there. Pushing the button Cancel Loading
correctly cancels loading (so the main app seems responsive), but
there's an empty project displayed with the disk title set correctly (as
in the saved project file).

Other project files happen to load correctly.

Malfunctioning project file is attached.

the fix is not really a fix but a workaround, like in Ubuntu:
   * Disable iptables and nut plugins on hppa as well to work around a FTBFS
 caused by #358637 and presumably #419684 (Closes: #430933).

Update:
After some investigation, switching from pulseaudio to native ALSA solved 80% 
of the problem.
Now, most of the time, Videos and Flash movies will play, but every 5th or so 
not. A page reload solves the symptom, but the bug is still there...

Environment: IBM x346 (Type 8840-2AG) with ServeRAID 7k (FW: 7.12.13),
Gutsy amd64 was installed, upgraded via do-release-upgrade to hardy 8.04
LTS.

Problem: Booting the recent kernel 2.6.24-19 (also -18) produces a system hang 
while initializing the ips driver. Console output is as follows:
ips: :08:0e.0: Reset Request - Flushed Cache
This message repeats until the machine is switched off.

Booting the 2.6.22-14 kernel from gutsy shows no problem at all, boots
happily into hardy and starts up the userland.

Correction: The package from intrepid just does not expose the crashes.
The icon in the notification area is still fragmented after running Psi
some time. The fragmentation is not deterministic, but varies in
patterns and time of occurence. So backporting intrepid's version is not
a real fix.

Apparently, this has been fixed in 0.11-8

After installing the package from intrepid, the QT4-dependent problems
seem to have disappeared.

I can second that (hardy/x86_64 as of 2008-05-08).
The corruption takes place after a certain amount of time.
It looks like the buffer for translucency of the icon fetches the background 
bitmap from a wrong mem address (my uneducated guess)...
