  I am trying to use a Toshiba external disk, in a usb3 port which I use for 
backup. It worked flawlessly on an old version of Redhat, but on moving to a 
new server, Intel S1200RP with updated firmware, I am in a world of pain. I 
have switched off standby for usb on boot, I have ensured that power is 
continually on for all usb ports. 
  I have tried a number of kernels, currently 3.19.0-031900rc4-lowlatency which 
had been less bad, but am still getting this fault.

This began when I migrated to a new server, the Intel I referred to, and
installed Ubuntu 15.04 on it. The standard kernel produced this error. I
tried many kernels. In the 4.4 and beyond series, including 4.5-wily, it
would not assemble or mount my raid array. So not much good there. But i
will test again. I am offsite at the moment so the apport does not
work...

i have ubuntu studio 12.04 and the same bug, except that terminal
emulators work in the x11 window, but other terminals do not. This
affects me since i want to have crontab begin playing music.

sorry, I should have said, pactl list for example works in a terminal
emulator i open from the xfce4 desktop, but gives me the connection
refused message if I do the same from a terminal outside that (same
user).

I also have this bug using ubuntu studio which allows realtime kernel use of 
the sound card. It is causing delays between playing sound tracks on a list 
kernel:  2.6.38-12-generic #51-Ubuntu SMP x64 I have checked my disc, it seems 
to be fine... there are no shared interrupts for the HDA
Some help would be appreciated.

I also have this bug. I have studio variant installed to provide realtime 
kernel access to sound card. Perhaps relevant.
Kernel is 2.6.38-12-generic #51-Ubuntu SMP x86_64.
checked discs as per other suggestions, all ok. I use the machine to broadcast 
music, and it is pausing between tracks in a play list...
this seems to be the only sign of a cause for the problem.
