[Bug 1429959] fix eeh on qla2xxx

The attached patch seems to fix all the problems we have seen, including when the adapter seems to have recovered, but can't do any IO. We have tested this on top of 4.0. Mauricio, can you build a package with this patch on top of Vivid's kernel, so this can be tested? Thanks.
