Sounds to me like it's time to report this bug to Apple. It is definitely a
bug, it is happening to me. Turning your Bluetooth headset off and back on
again does resolve the issue. Same with a Bluetooth keyboard.

I have found this bug too. Telling my phone to forget the blue tooth headset
and then pairing again worked to get VO through headset but is a pain. Harry

Yes it would seem that the new audio destination rotor setting is not working
as it should.

re boot my i pad mini then it will only last for about a half hour
