Does this happen all the time? Do you ever hear VO through the iPhone speaker?
The only thing I can think of is to check the setting for routing
in-coming calls under accessibility. Make sure it's set to default.

Audible, radio apps, etc all behave as normal, Siri as well, but VO is being
routed through the earpiece the second I unplug my headset. This is a newly
replaced phone, and I'm positively perplexed.
