You can use PayPal I believe as well as iTunes gift cards. The gift card thing
is helpful say if you've are part of a family and you want to make in app
purchases with games or whatever and not have to deal with it getting charged
to the main account holder's method of payment, but rather have
I'm pretty sure you can, at some point during the set up you should be able to
select to add a card later or no card.
I also haven't set up an Apple Id for a long time, but the process should be
quite self-explanatory and accessible.
