The headphone and speaker have separate volume controls. With the headphoones
plugged in, turn the volume up. Once you set these controls, they won't be
modified unless you change them. Whatever is sounding will change when you
press the volume

Sounds like the volume of VoiceOver got turned down on your internal speaker.
Get VoiceOver to start speaking, then increase its volume.

your screen dims, or locks, you may not be able to read with voiceover as you
did. You might want to turn off that place in settings that talks about screen
locking.
