I did all that, my sighted husband did all that, trust me it wasn’t  that my 
volume was lowered. something was stopping my audio from routing to external; 
it was even grayed out in the audio settings in accessibility. it was bizarre.

Hi Alia,

If volume drops to 0 you can get it back relatively easily if you follow a few 
steps. If you still can use headset then it's even easier. Just use the headset 
and make sure the phone is unlocked and you are at the top of your home screen. 
Now unplug the headset and flick up or down with two fingers, this will start 
Voiceover reading continuously. Now press the volume button repeatedly, it's 
important to press it at least 5 or 6 times and not to press and hold it. At 
that point you will start hearing Voiceover again.

It just Lord your volume all the way to zero. You just have to hit the up 
volume key a bunch of times. It seems to me that it only does this upon initial 
use, although I'm not sure. It didn't do it on my iPad.

Well it could've been a one time thing. But what I changed in settings was the 
default which is to have it start playing as soon as the app opens, so I change 
that off. And mute when the app opens which I believe is set on, and I changed 
it off. Basically I changed any settings that I thought might affect voiceover. 
And I have not had that problem since. One good thing about the iPhone app is 
that a two finger double tap actually will shut the channel up if you need to. 
On the other hand, I think the iPhone app app is so screwed up that I can't 
imagine actually using it for anything. Maybe if I only had a very few favorite 
channels, and could figure out a how to add them, and B, how to change from 
Alta favored, I might use it. But as it is now, not going to happen. The Apple 
TV app, despite its major shortcomings, appears to be the best, because you can 
reliably move from the button where you would add it to a favorite to the 
button where you would play the channel to the button where it's going to 
announce the program, although it doesn't tell you the channel. So, if your 
memory is really good, or you don't like but a few channels, the Apple TV app 
could work for you. And then you get to ask AT&T for a discount, because it's 
really not accessible ha ha.
