over use and now makes a very dodgy connection to the cable modem
resulting in no data flow.
Can this be repaired or is a PMCIA card the most cost effective option?

Although I can't imagine why your startup items are being difficult,
it could be a permissions problem. Startup items are recorded in
~/Library/Preferences/loginwindow.plist
