Would you mind elaborating on the first point? I vaguely recall test
banners being shown to logged in users, but don't recall seeing one myself
while logged in for a while.

Unfortunately, while there may be multiple gadgets / features exhibiting
this undesired behaviour, it is not a general problem or limitation. As
such,
