We just use the standard roof racks, but they even have gas shock lifters on
some.

We use bungee cords, preferably the kind with the carabiners on each 
end. One in the front and one in the back. They have never failed. Only 
people have caused failures so far.

I know Dish/Direct/whoever have the vans with the ladder rack that slides
off/down a bit - perhaps find one of those on ebay?

The make really good roof rack ladder mounts, complete with lock downs and
quick releases.  Take a look at the Earl Owen Company website.  They've got
all sorts of good stuff.
