make sure the file is at least chmod'ed to 111
aka : chmod 111 antivirus.sh
Make sure you restart the email server after installing the filter
Make sure you have service clamd running.

Problem with this would be that it is xmail that reads and processes the
server.tab file, little late to say what user it should run as.
