Well, I'm happy to report that generating WebHelp this morning with the
parameter ignore-navigation-links=no gave me the output I wanted. It must have
been some time since I last generated that particular Help set :-}

* the name of the element is vrmlist
* the order of the attributes (driven by the alphabet/DITA standards,
so nothing we can change there) is mrv
* the Prolog

Happy to pass on a question from a team of developers: how can they take the
stack of .dita files (plus .ditamap, document conversion parameters in an .xml,
and the referenced graphics I use) and use a command-line to generate the
WebHelp?
I'm pretty sure there is a simple "Yes; use ..."
