THE MISSING LINK
Computer report exaggerates deaths in Michigan

The HAL 9000 in "2001: A Space Odyssey" was a sweetheart compared to the computer at Saint Mary's Mercy Medical Center in Grand Rapids, Mich. The computer proclaimed 8,500 patients dead -- news that came as quite a surprise to those people who were not wearing toe tags and had all their vital signs. So what happened? A computer glitch, of course. Evidently, during a file update an important digit was dropped -- the one that distinguishes between "deceased" and "discharged to home." That's a pretty big digit.
