When I login via ftp localhost I challenged for a userid/password and I can
successfully login, however when I ftp externally i.e., ftp rh71server I
immediately get the connection refused message.  I never get a chance to
specify a user or password.

That means that you have no firewall.  Have you actually logged in via 
`ftp localhost`?  Did you use this same user/passwd on the remote machine?
