I see nothing at the HiVal site to indicate this company has sought 
bankruptcy relief.  There are company press releases onsite and not a 
thing about bankruptcy or any other problems:

Their telephone number is posted.  It is long distance, but for peace 
of mind you could call them for an explanation of the correspondence 
you received.

Quite possibly it is just a case of a third party -- a contracted 
company, handling the rebate administration, or else the middle man 
that sold the product to you is responsible for rebate management.

...and...I just took a peek at a pdf file of what I believe might be 
the last rebate and it expired on 2/28/00.  Maybe that is what your 
letter was trying to convey.
