Ana - it does appear that you created it, although you probably weren't
aware of it at the time; see here:

It also appears that this was copied from Wikipedia's version. There's no
reason, though, why Semantic Forms shouldn't be able to work with this
Javascript page; what's more, it appears that it did before the latest
version, 1.6; it was probably the change that allows for setting a default
destination filename that's causing the error. So we can call it a bug in
SF.
