I hope you mean that all extensions would still have their own sub 
directory under the extensions directory?

What I do now for all MW installs I set up, is to simply run
svn checkout http://svn.wikimedia.org/svnroot/mediawiki/trunk/extensions/

and so I have everything in place. For this I use a local
...\extensions\ExtensionsSettings.php

in which I include the extensions I want to use. Simple. And no, I don't 
think using up disk space is what would keep MW administrators awake at 
nights. :)
