the RFC is not written with a B2BUA in mind. For a B2BUA, it makes
sense to try to get out of the refresher role. Therefore sems doesn't
put 'refresher' parameter there. What you mustn't do is to put
refresher=uas there if you are uac and have the refresher role (afair).
