Interesting that you use NICOM and PICCO for monitoring in septic shock, since there are almost no robust data supporting the use of these two monitors in sepsis. The use of CVP is often misinterpreted. CVP is only helpful if it is LOW. Then, it can be used as a therapeutic target in resuscitation.

Marik Dr Marik is one of the attending physicians at my facility... If anyone has specific questions please send them to me and I will track Dr Marik down... I am not sure if he follows this list serv or not however I am going to forward him a link to it and attempt to get him involved. I have not reviewed this specific article in question but will later this evening... However I do know that Dr. Marik is not a fan of CVP and has lectured on the inconsistency of CVP monitoring as well as published a meta-analysis article on the issue. We do use NICOM and PICCO monitoring on our septic shocks patient in lieu of CVP. Much to my chagrin, it contained unfavorable reviews of the SSC bundles. Hoping to hear from you all, Carina MD. USA PS Since the NICE-SUGAR study, is the majority still using intensive insulin therapy drip for hyperglycemia in the ICU patients? Our pharmacist shared an article (details & source to follow, apologies) regarding the use of insulin drip to treat hyperglycemia as a weak recommendation.
