Time 5.Sepsis targets achievement rate in percentage and time of achievement. 6.Source control 7.Mortality rates 8.Whether early adequate therapy was correct? 9.Whether source of sepsis was correct?

This does not capture all the sepsis patients in the hospital since some patients are not that unwell to require outreach nursing care.
