Dr. Ranganathan - Depending upon the charting system you are currently using 
within your facility, there are numerous ways to "capture" this data and 
monitor it concurrently.
Currently our facility uses Medi-tech (just another word for ancient I'm 
certain); however, it does allow me to run reports to capture any patients who 
may have been discharged with a diagnosis of Severe Sepsis and/or Septic Shock. 
I also review patients daily who have had "positive" sepsis screens, lactic 
acids and/or procalcitonins drawn.  I keep a file for all inpatients who I am 
watching concurrently and update it daily with information pertaining to the 
goals of our Sepsis Bundles. At the end of the month we have Division calls to 
compare our data and see what is or is not working in the different facilities.
We've decreased our overall mortality by almost 40% since last June alone, and 
we continue to make progress daily.

Dear all,

Does any one know how to audit/monitor the care given to sepsis patients.

My hospital is asking us to monitor and report back the sepsis care. Presently 
we do not have any system to do this. We take info. given by outreach nurses 
(who are all ICU trained) to capture sepsis patients currently. This does not 
capture all the sepsis patients in the hospital since some patients are not 
that unwell to require outreach nursing care.

I thought of asking all of you, especially people from UK if you have any 
system/way to capture all the sepsis patients in your hospital. I remember Ron 
Daniels replied something along this line in another e-mail few months ago.

I would be grateful for your reply.
