Some of the physicians I work with believe that the initial fluid bolus of 30 
mL/kg should be based on ideal body weight, not actual body weight.  Since the 
guidelines don't specify, am I correct in assuming that we should be using the 
actual body weight?  I could not find any literature on this specific subject.
