That's cryptic shock. Elevated lactate =end organ dysfunction and should be 
taken seriously. Central line indicated. Resuscitate and recheck to follow 
trend. 

Define normotensive?  80 yo w baseline sbp of 160 who 
 presents with sbp of 115?  Interpret with caution.
