I exclude anyone that is d/c to inpatient hospice, any patient that comes in as a comfort measures only, a patient who has died within 24 hrs, and any patient who states they do not want aggressive measures (this must be well documented by the provider). We do count all DNR and limited therapies. Hope this helps you
