Anyone willing to share how you do sepsis mortality review at your facility?
Any articles or presentations or software you could recommend?
We are looking to start doing 100% review of all sepsis patients in our 
mortality review.
Do you have independent review by  both a physician and a nurse?  Or just one?
Do you have a larger group meeting to discuss the results?
Your suggestions are welcome.
