Accurately finding severe sepsis presentation time several days into the 
hospital stay has been very tricky to establish. My scenario is....

Pt admitted with no signs of infection through the ED, No +SIRS. Day 2 of 
admission urine culture came back + for E-Coli at 10:53 first sign of 
infection. Pt does still does not have 2+SIRS. Pt goes to surgery for a 
cholecystectomy and 2 days after surgery the patients LA comes back at 0800 is 
2.1, with 2+SIRS.  Does the infection that was diagnosed earlier in the 
admission  stay with the patient 4 days after + cultures? With the LA 2.1 this 
would put the patient in the category of end organ dysfunction=severe sepsis, 
so the 3 hour clock would start when the LA results come back, and I would look 
3 hours prior to 0800 and 3 hours after 0800 to make sure the 3 hour bundle was 
completed? a repeat LA within 6 hours?

Any help with this would be greatly appreciated.
