Just to clarify, does the 3/6 hour clock start when the patient meets severe 
sepsis "clinically" (test results, vitals) or does it start when it is 
"Documented" by a physician or midlevel  that we are now calling the patient 
severely septic (physician progress notes, admitting diagnosis, etc.
