I have a report card, but it is for the organization, not an individual department. We found the N is not big enough for any one department other than CCU and ED. But I bet that would have more of an impact to the individual unit! I will attach a copy, knowing that attachments are problematic for this list serve, but at least Amy should get it? Basically, it trends each element and the overall "all appropriate care" compliance rate. This one page is created in Tableau, and posted on each unit. I have a separate dashboard in Tableau for the Sepsis PI team I facilitate, which includes other data like Mortality, by Payer, and fallouts by unit.
