QUESTION:  If the patient doesn’t have documentation of severe sepsis nor 
septic shock, BUT does meet severe sepsis criteria (EX: Pneumonia, 2 SIRS and 
Organ dysfunction ARF or LA 2+), you would have to abstract the hypotension and 
treatment correct?
(In the meantime, remember that if the MD documents within 6hrs of TOP that the 
patient does not have Severe Sepsis or Septic Shock, then you would not 
abstract this. )


In addition, if prior to TOP Severe Sepsis/Septic Shock or within 24hrs after 
TOP, the MD documents the hypotension is related to something else NOT an 
infection/sepsis OR is normal for the patient, then you would not use the 
hypotensive BP for the element of initial hypotension.
