We are having issues with providing the full 30mL/kg volume for septic shock in patients with either End Stage Renal Disease (ESRD) or CHF. Does anyone have a strategy that worked for you that convinced providers the patient still needs the volume in the presence of septic shock? Or is it known if there is any movement by CMS to allow for exceptions for patients with these comorbidities?
