Our sepsis screening is part of the shift assessment, so it's definitely done qshift. There is a standalone screen as well that can be done PRN patient status change. Positive screens in our facility require the nurse to immediately call a rapid response (even in the ICU), which may or may not be followed by the overhead page "sepsis alert," it's the same people who come to both. If I'm in house I get a page with every rapid response so I'll respond as well. Whenever the sepsis bundle order set is utilized I get a printed notification, sometimes the ED will do this without calling a rapid/sepsis alert.
