I agree with Mary. The headings are not acceptable to take.
I do not see documentation in the body of the ROS that 'it was completed or 
reviewed' as a provider statement. The Qnet response was to use a statement 
documented as I understood it.
For physical examination, we need to have all 5 elements and I can only see one 
(VS)

I know that we can't use it if it's in a header, but would these be considered 
headers?
