We use the CMS guidelines (SIRS, organ dysfunction, and source of infection) for early recognition of sepsis, which works well for this and then we use the qSOFA for a decision point as to what unit the patient should go to. For instance: all of the septic shock patients come to ICU but should they have severe sepsis and a qSOFA score of 2 or > they go to the ICU step down unit where they are monitored more closely. If the qSOFA score is < 2 and they have severe sepsis they can go to the medical unit. All of the research I have done on the qSOFA/SOFA score shows that it isn't necessarily good for early recognition but more for risk of mortality. I hope this helps?
