1. Are you screening every patient?
we screen every patient 18 and older, maternity has a slightly
different screen. Pediatrics follow PALS recommendation
2. Are they screened in Triage or not until they are in a room?
In triage, q 12 hour, prn, and with transfer
3. If not screening every patient, what are the triggers for screening?
2 SIRS, s/s infection.
4. Is the screen on paper or electronic?
Electronic; we have 2 computer systems and both have
screen mechanisms.

Good Afternoon,
We thought the new lactate rule would be helpful, but had our first miss of
2019.
Time zero: 0636
First lactate sent 0227 (2.9)
Second lactate sent 0445 (2.0)
Last year this would have met the metric because the abstractor would use the
lactate sent closest to time zero (result was 2.0; no repeat needed).
This year the lactate with the higher value is used (result 2.9). But, the
lactate sent at 0445 can't be used as 'repeat' because it was sent before time
zero.
Are any of your systems sending 3 lactates instead of 2, even if the repeat is
normal? Any other suggestions?

RNs are screening at SFH when triggered by the presence of 2 SIRS. The EMR
begins screening and sends an alert when 2 SIRS criteria met in the EMR. The
pop-up alert requires the RN to answer whether the patient has a suspected
infecti0n.
If yes to infection, a severe sepsis screening flowsheet automatically opens.
This is instituted hospital-wide and screening takes place whether in Triage,
Main ED, or in-patient areas if 2 SIRS plus infection are present.
