We reflex at 2 hours as well; basically to account for any delays that may occur in collecting lab at peak times/late hours with limited staff, etc. We have found it to be very effective. We do still have problems with the timing of LA like when one is ordered really early and a second one reflexes but based on time zero the second one becomes the first. We're discussing the possibility of reflexing a third if the second is still >2 to see if that helps. They have to be ordered stat which on occasion is problematic as well when the patient is outside of the ED setting. We are trying to convert to ePOC LA but aren't using it at even 30% across the system at this point.
