Ah - I have at least one of the controller working now - and have maybe learned a lesson....

I noticed that I was using my actual user data database as the recovery database - it seemed to be creating the RECOVERY et al tables in that database ok - but deleting these tables didn't fix the "not last man down" problem.... So I created a different database for the recovery stuff anyway, as per the Sequoia examples. Now I notice that I can start the controller ok. So perhaps a very good idea to give the recovery tables their own database!
